It's called the "Ezo Squirrel Omikuji." Inside a palm-sized ceramic Ezo squirrel (Hokkaido squirrel) is a fortune slip. The offering fee is 500 yen. When you insert a 500-yen coin, a capsule containing the squirrel figure appears. There are four types in total—red, yellow, blue, and a secret one—so which squirrel you get is a surprise.
